Treatment Schedule

Office Policies

Payments: Cash, Check, Visa, and Mastercard

Confidentiality: All patient information is held in strict confidence as required by law.

Cancellation Policy: When you make an appointment the time is reserved for you. If a cancellation is absolutely necessary, 24-hour notice must be given or our usual fee will be charged. Please leave a message on our voice mail if calling during non-business hours.

Return Policy: Unopened bottles of prepared Chinese Medicinal may be returned for a full refund as long as the bottle has not yet expired or will not expire within the next two months. There will be no refunds given for any raw herb as this is a custom made formula for you. It is very difficult to separate all the herbs after they have been mixed together (and some herbs are crushed, grounded, etc.). Also, once the herbs leave the office, they are considered “contaminated” and can not be used for another patient.

Our staff policy is to be kind, courtesy, and to give you the best treatments possible. It is through our satisfied patient’s referrals that our practice has grown so well.

Initial Intake Forms

You are encouraged to complete the relevant form(s) and bring them with you to the office. This will ensure that during your first visit to our office more time will be spent with the practitioner (instead of filling out paperwork).
